Visit us at www.nhcc.us What We Do The Care Coordination Program provides services as designed by the NYS DOH Health Home Model, based on unified systems of care to coordinate and integrate physical and behavioral health care, substance use treatment, and social services. The goal of Care Coordination is to minimize the use of hospitals and emergency rooms by providing access to all of the services an individual needs to assure he or she stays healthy.
