Health Quality Partners (HQP) is a not-for-profit organization focused on R&D to enhance the effectiveness of preventive interventions and improve population health in vulnerable, higher-risk populations. Through their model of Community-based Advanced Preventive Care, HQP has demonstrated significant reductions in hospitalizations, deaths, and Medicare expenditures for older adults. The organization is currently seeking strategic partners and early adopters for their innovative approach to preventive care services. HQP plans to establish a learning network to accelerate applied R&D for population health management in 2022.
